A 52-year-old man, offspring of a consanguineous marriage, follow-up study for Morquio's syndrome, presents with progressive generalized dystonia, gait, speech disturbances, bilateral hip pain, and paraparesis. His brother has similar symptoms.
A 78-year-old man, status: post-exploratory laparatomy and small bowel resection. Following removal of central venous catheter, patient found to be unresponsive, with dilated pupils.

Lab evaluation c/w secondary hyperparathyroidism with normal serum calcium (10.0mg/dL), elevated parathyroid hormone (142pg/mL; normal 10-60pg/mL), and low vitamin-D-25-OH (15.2ng/ml; normal 31-80ng/mL).
