Chronic Inflammatory Demyelinating Polyneuropathy (CIDP)
- CIDP is an acquired disorder of the peripheral nerves and nerve roots.
- The brain can be involved, with lesions appearing similar to those seen in multiple sclerosis (MS).
- Clinical Presentation: Symmetric motor involvement is greater than sensory. Weakness is present in proximal and distal musculature, with slow progresive course.
- CIDP can affect all ages but is more common in older men.
- Pathogenesis: Likely immune-mediated, with multiple triggers
- Key Diagnostic Features: Enlargement of nerve roots and peripheral nerves. In the brain, periventricular lesions similar to those observed in MS are seen.
- DDx:
- Thickened, enlarged nerve roots: Lymphoma, sarcoidosis, neurofibromatosis type 1
- Brain lesions: MS and other infectious and inflammatory disorders
- Rx: Immunotherapy
