Brainstem PRES
- Rarely, posterior reversible encephalopathy syndrome (PRES) presents with predominantly pontine involvement.
- Differential diagnosis includes brainstem infarction, central pontine myelinolysis and astrocytoma.
- A lack of diffusion restriction, presence of posterior subcortical white matter involvement and appropriate clinical presentation favor PRES.
- Immediate medical management of underlying etiology is necessary to prevent more serious complication of infarction.
